Kindly, i do not understand the section: "I understand that it is
the sole responsibility of Prize winners of US$600 or more to provide certain information to facilitate receipt of the award,"

regards.

    .. including completing and submitting tax forms. It likely means Samsung will ask for a simple tax form (I-9 or equivalent) before they pay you. It's usually not a difficult one.

    I also have a question about the documents. I'm a single member LLC and on my taxes that is treated as a "disregarded entity". As such, my taxes for the LLC use my SSN over the EIN assigned to the business (see: http://www.irs.gov/Businesses/Small-Businesses-&-Self-Employed/Single-Member-Limited-Liability-Companies ).

    For the affidavit, would I provide my SSN as that is what will be on my tax return documents? Or is the business EIN still best for what to put there?

    Thank you for your question and congratulations on being named a finalist. The section of the affidavit which states "I understand that it is
    the sole responsibility of Prize winners of US$600 or more to provide certain information to facilitate receipt of the award, including completing and submitting any tax forms when necessary and any applicable withholding and reporting requirements," refers to the additional tax-related forms which you will receive from Samsung after you have completed and returned your affidavit. You will need to fill out these tax-related forms to receive your prize.

    Hi.
    I made my app myself, without team. So must I fill the second document (for teammates) or it will be enough to fill just first (for team representative)?

    Thanks for your question. If you are the only person who worked on your app, you will need to fill out the affidavit for Team Representative.
